Are firearms the leading cause of death for children? What CDC numbers actually show
Both things can be true: on broad pediatric bands (especially 0–19 / 14–19), firearms lead recent CDC WONDER injury rankings — and for younger children the ranking collapses. The slogan’s truth depends on ages, comparison causes, and intent. Here are the numbers.

Methods & limits
Question. When someone says firearms are the leading cause of death among “children,” which CDC-based numbers make that statement true, which make it false or incomplete, and how do age, motor vehicles, and intent change the picture?
- Beacon rule: we do not win by hiding inconvenient CDC totals, and we do not lose by letting undefined words do the work. Every ranking below names the age band, the comparison, and the source.
- Primary age-split rates and 2023 counts: Roberts et al., Injury Epidemiology (2025), open access PMC11931740 — authors queried CDC WONDER for ages 0–19, 2014–2023, ICD-10 injury mechanism/intent. They report firearms as the leading cause for ages 0–19 in 2021–2023 (firearm crude rates 5.8 → 5.7 → 5.5 per 100k; MVT 4.7 in 2022 and 4.9 in 2023). For 2023 they report 4,470 firearm deaths vs 3,967 motor-vehicle traffic deaths at ages 0–19.
- Younger vs older split (same CDC WONDER paper): cumulative firearm deaths rise sharply around ages 13–14. For ages 0–13 in 2023, leading traumatic causes were suffocation (2.8), motor vehicle traffic (1.8), drowning (1.3); firearms were fourth (1.9). For ages 14–19 in 2023, firearms led (15.0), then MVT (11.4), then poisoning (6.6). Firearms became #1 for 14–19 in 2016 in their series.
- Intent mix (same paper, 2019–2023): homicide was 59.6% of firearm deaths ages 0–13 and 65.3% ages 14–19. Suicide was 19.7% (0–13) vs 30.8% (14–19); 92.5% of pediatric firearm suicides were ages 14–19. Unintentional share was 15.2% (0–13) vs 1.8% (14–19). So the broad gun-deaths total is mostly intentional violence and self-harm concentrated in older adolescents — not a uniform accidental-risk story about small children.
- Late-teen inflation (KFF analysis of CDC data, 2021): ages 1–17 firearm rate 3.7 per 100k (~2,571 deaths); ages 18–19 rate 25.2; ages 1–19 ~4,733 firearm deaths. Including 18- and 19-year-olds (legal adults) roughly doubles the 1–17 count and multiplies the rate.
- Under-18 vehicle comparison (Crime Prevention Research Center, using CDC 2024 and FBI murder series): constructing under-18 firearm deaths as FBI murder + CDC suicide/unintentional/undetermined yields 1,893 vs 2,193 CDC motor-vehicle traffic deaths (and 2,239 broader transportation). That shows how under-18 + vehicle comparator + murder-vs-homicide choices can reverse a headline. We label CPRC’s construction separately so it is not confused with pure CDC homicide totals.

- What truth requires in prose: (1) If you mean ages 0–19 or 14–19 with injury-mechanism rankings, recent CDC-based analyses really do put firearms first. (2) If you mean younger children (0–13), firearms are not first. (3) If you mean kids as ordinary speech, you must say the ages. (4) Intent and vehicles are not optional footnotes.
- Future pull (queued, not claimed tonight): golf-cart / similar nontraffic transport accidents vs AR-15 / rifle deaths — only with defensible ICD coding and weapon-type definitions on both sides.
Limitations. This page still relies on published CDC WONDER queries (peer-reviewed + KFF + CPRC), not a fresh LEE-owned bulk WONDER extract in S3 yet. Rankings depend on mechanism grouping (injury mechanisms vs NCHS 113 rankable causes), provisional vs final files, and whether age 0 is included. CDC homicide includes some justifiable homicides; FBI murder does not treat those the same — CPRC’s under-18 comparison uses that distinction on purpose. Racial and geographic concentration is large in the literature and not fully tabulated here. Suicide method-substitution debates are noted by CPRC but not adjudicated here.
